JOB DESCRIPTION

Provides direct case management services to older adults in Baltimore City.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Perform initial assessment/investigation of alleged disabled adults in hospitals, nursing homes, assisted living facilities and/or private homes
  • Attend court hearings
  • Providing long term case management to clients in various living environments
  • Arranging for and consenting to appropriate medical care and treatments
  • Maintaining case records and notes
  • Maintaining appropriate data in various data tracking systems
  • Preparing and presenting reports at the Adult Public Guardianship Review Reports as required by Guardianship Statute
  • Providing after hour and weekend coverage on a rotating basis
REQUIRED QUALIFICATIONS
  • Interest in serving older adults and persons with disabilities
  • Time management and organizational skills
  • Ability to grasp laws, statutes, program policies and apply them appropriately
  • Ability to learn legal and medical terminology
  • Good computer skills as well as good written and oral communication skills
  • Ability to work effectively as a team member with co‑workers, physicians, attorneys and other professionals

Hybrid work schedule with field visits to facilities as well as office days in addition to remote work.

Some local travel required.
