Community Outreach Liaison
The Community Outreach Liaison is responsible for generating qualified referral leads to support occupancy goals at their assigned Northbridge communities. This role builds and strengthens relationships with healthcare, professional, and community referral sources through consistent outreach efforts, including in‑person visits, presentations, event coordination, and follow‑up communication. Working closely with community and home office leadership, the Community Outreach Liaison supports outreach planning while ensuring Northbridge brand standards and service expectations are consistently represented.
EssentialJob Functions
- Outreach & Referral Relationship Development
- Develop and maintain strong relationships with professional, healthcare and community referral sources to increase qualified referral and improve lead generation outcomes.
- Conduct consistent outreach activity, including cold calls, in‑person visits, presentations, and structured follow‑up communications.
- Act as liaison between assigned communities and professional referral sources, ensuring contacts are made in a focused and prioritized manner.
- Proactively identify and pursue community‑based publicity and outreach opportunities that support referral and move‑in activity.
- Actively participate in local groups, networking organizations, trade groups and professional associations.
- Event Planning
- Support coordination of community‑based outreach events, seminars, education programs and functions to increase brand awareness and referral development.
- Partner with community leadership to plan and execute events that align with occupancy, sales and move‑in objectives.
- Represent Northbridge and assigned communities in a consistently professional manner.
- Documentation & Reporting
- Maintain accurate and timely referral source and outreach documentation, including contact notes, communication, and follow‑up plans using CRM.
- Maintain accurate and up‑to‑date database of professional, healthcare and community referral sources.
- Prepare outreach activity updates, plans, reports and professional correspondence as needed.
- Ensure referral activity goals are consistently met, including required referral contacts as directed by Executive Director and/or VP Marketing.
- Associate’s degree or Bachelor’s degree preferred in Marketing, Communications, Business, Healthcare Administration, or related field. Relevant work experience may be considered in lieu of degree.
- Minimum of 5 years of experience in community outreach, healthcare marketing, referral development, sales support, business development, or similar role.
- Experience working in senior living, healthcare, home health, hospice, rehabilitation, or case management referral environments preferred.
- Must possess a valid driver’s license and maintain reliable transportation.
- Must be able to travel frequently to assigned communities and referral sources within the designated territory. Requires the ability to communicate in English both orally and in writing.
- Ability to communicate effectively with residents, families, staff, community officials and the public.
- Must have compassion for and desire to work with the elderly.
- Ability to relate to the work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the community.
- Ability to work a flexible schedule, including occasional evenings and weekend based on business needs.
- Must meet all State health requirements.
- Must pass criminal background check.
