• Participates in patient admission process (Inpatient, Outpatient, PHP, IOP) and assists as needed with signing of voluntary papers; serves as hospital representative and patient advocate.
• Coordinates and facilitates behavioral health services for patients and their families throughout the Stormont Vail Health system, including but not limited to:
  Cotton O’Neil Clinic settings, Intensive Outpatient Program, Psychological Services, Outpatient Psychiatric Department, Outpatient/Inpatient Behavioral Health Services, or Behavioral Health Admissions Department.
• Performs social work assessments, considering biological, psychosocial, cultural, developmental and spiritual aspects of illness and hospitalization.
• Develops appropriate case management and/or treatment plans based on assessment and multidisciplinary care team recommendations.
• Addresses patient/family/caregiver understanding of and adjustment to illness to maximize benefits of intervention and enhance functioning.
• Initiates discharge planning upon admission; coordinates plans with internal and external multidisciplinary teams; develops long-term planning to reduce recidivism; makes referrals as indicated.
• Develops and facilitates groups for patients and/or significant others as needed.
• Maintains professional productivity; coordinates and prioritizes patient care activities efficiently and effectively.
• Demonstrates initiative and provides problem solving and leadership in developing new services to respond to patient care demands.
• Maintains confidentiality and complies with professional ethics according to professional, departmental and Medical Center standards.
• Participates in inpatient care conferences and provides patient education regarding diagnosis and treatment.
• Provides crisis and/or brief therapy for individuals, families and groups.
• Assesses patient and family functioning at discharge or when referred to another provider.
• Communicates clearly with patients, families and caregivers about status and progress; adjusts communication style as needed.
• Establishes supportive and therapeutic working relationships with patient/family/caregivers.

• Licensed Bachelor Social Worker (KSBSRB) – Temporary licensure may be accepted; full licensure must be achieved within 6 months of hire. Required
• Licensed Master Social Worker (KSBSRB) – Temporary licensure may be accepted; full licensure must be achieved within 6 months of hire. Required

• 1 year Medical experience. Required

• Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
• Performs other duties as assigned

• Workspace is quiet and distraction-free, allowing compliance with security and privacy standards.
• Stable electricity and minimum 25 Mbps upload and internet speed.
• Dedicate full attention to job duties and communication during working hours.
• Adhere to break and attendance schedules as agreed with supervisor.
• Abide by Stormont Vail’s Remote Work Policy and review Remote Work Agreement annually.
