Primary ResponsibilitiesThis is a contract (1099) role for a Hybrid Therapist with Well Home Psychology with sites in Merrillville and Fort Wayne, Indiana. Positions blend flexible remote work with in-person clinical services delivered throughout 10 NWI counties (Lake, Porter, LaPorte, Jasper, Starke, Pulaski, Fulton, White, Cass, and Carroll) and the Greater Fort Wayne area

Hybrid Therapists provide individual, couples, and family therapy using evidence-based practices within Well Home’s trauma-informed, community-based model of care. This role includes developing and implementing individualized care plans, maintaining effective communication with clients and relevant stakeholders, and supporting continuity of care across settings. The therapist is responsible for maintaining timely and accurate clinical documentation, adhering to ethical and regulatory standards, and participating in required consultation, supervision, and team collaboration.
Core Responsibilities:
- Deliver clinical services through a combination of virtual, in-home, and community-based therapy
- Complete clinical documentation accurately and on time using the practice EHR
- Participate in consultation and supervision as required, particularly for pre-licensed clinicians
- Administer, review, and integrate outcome measures and screenings in accordance with practice SOPs
- Collaborate with care coordinators and leadership to support client engagement and continuity of care
Practice in accordance with ethical standards, state regulations, and Well Home Psychology clinical policies

Minimum Qualifications
• Master’s or Doctoral degree in Psychology, Social Work, Counseling, or related field
• Active Indiana license or license-eligible under supervision (e.g., LCSW, LMHC, LMFT, Psy.
D., Ph.D., or associate status)
• Ability to work independently and manage clinical responsibilities
• Comfort providing therapy outside of a traditional office setting
• Reliable transportation for in-home or community-based services
• Strong communication and documentation skills
• Experience with trauma, anxiety, mood disorders, or neurodivergence
• Experience with children, adolescents, or families
• Interest in community-based or experiential therapy models
• Training in evidence-based modalities (e.g., CBT, DBT, EMDR, TF-CBT)
• Military, veteran, or underserved population experience
• Compensation is determined after the interview process based on licensure level, experience, and role alignment
• No salaries are guaranteed or disclosed during the initial hiring phase
Well Home invests in clinician growth through optional training support and supervision. Some advanced trainings may include shared-cost agreements to balance professional development with practice sustainability.
