Overview
Inpatient Case Manager/Social Worker, MSW – Days/PRN for the Resource Pool.
Location:
Suffolk, VA. Multiple shifts available.
Required to work at four hospitals in the Southside or Western Tidewater regions. Location and unit to be assigned based on greatest need. Must be within 75 miles and 90 minutes of all four hospitals in the assigned region.
Responsibilities- Provide medical social work services to patients in the hospital setting.
- Assess social, psychological, cultural, environmental, and financial situations, as well as disposition needs of referred patients.
- Collaborate with patients, families, healthcare team members, and community organizations; use advocacy skills to identify needs and resources in the hospital and community.
- Develop and evaluate transition plans considering diagnosis, prognosis, treatment, services, goals, provider options, and available health care benefits as appropriate to the continuum of care.
- Be flexible in practice settings:
Hospitals Only.
- Master's degree in Social Work required
- Certification in Case Management (CCM, CMAC or ACM-SW) required within one year of eligibility.
- BLS required within 90 days of hire.
- One year of related experience.
- Willing to work shifts at any of the four hospitals within the region based on greatest need; within 75 miles and 90 minutes of all four hospitals.

- High degree of flexibility for hours and shifts; regional and/or local travel; additional pay incentives.
- Operational hours: 8:00 AM – 4:30 PM, 7 days per week. Flexi hours: prescheduled time minimum 48 hours per calendar month, with 16 hours including weekend availability. Self-scheduling via API.
