Overview
Sentara Quality Care Network in Harrisonburg VA is looking to hire a Remote Part-time Social Worker, MSW. This is a remote position; however candidates must reside in the Harrisonburg area and surrounding areas as travel is required.
Key ResponsibilitiesAs a member of the Population Health Team, the Social Worker, MSW, will contribute to results as defined by the principles of value-based care. The position will collaborate with and serve as an expert consultant to the Population Health RNs, providers, and practice staff to engage in management approaches that optimize health outcomes. Responsibilities include acting as an advocate for the patient (beneficiary, member), family and/or significant others, and serving as a liaison between providers, patients, care team members, skilled nursing facilities, home health, hospital staff and community partners/resources.
Act as a resource to assist patients in need of community-based program assistance and empower the patient with the skills to navigate the process for obtaining appropriate medical care and community assistance.

Education
- Masters in social work required.
Experience
- 2 years' experience in direct patient care and/or case management required.
- Experience working with the frail or elderly population preferred.
- Knowledge of Medicare, Medicaid and CMS regulations.
