Overview

Band 5 staff nurse
- The post holder will work to support Public Health Nursing Children's Service to ensure the sharing of high quality, relevant and proportionate health information and attending multiagency statutory strategy discussions for children to contribute to the analysis of risk and planning. This also involves sharing relevant health information for social work assessment.

Responsibilities
  • Work alongside and develop relationships with colleagues and partners to ensure a high quality and consistent approach to strategy discussions, in adherence to the statutory Guidance Working Together to Safeguard Children.
  • Establish positive and supportive working relationships with partner agencies, ensuring the childrens services perspective is represented at strategy discussions.
  • Review the electronic health records of all children & parents (where relevant) for whom strategy discussions will be held ensuring that relevant and proportionate health information is collated for sharing within the strategy discussion. This could involve providing & listening to complex and sensitive information.
  • Work in an agile & mobile manner to support the completion of strategy discussions, whether that be attending virtually or by telephone to contribute to the discussion.
  • Share relevant and proportionate health information about a child into the strategy discussion or for the social worker assessment
  • Maintain contemporaneous record keeping in each persons electronic health record providing a brief synopsis with outcomes & health actions from the strategy discussion.
  • Escalate any concerns regarding risk or exceptional circumstances where multiple, unrelated children are subject to strategy discussions.
