The shift pattern for this post is between Monday Friday 09:00 - 17:00.

About the TeamInverclyde Older Persons Mental Health Team (OPMHT) is an integrated health and social care team providing a compassionate, culturally sensitive and caring approach to adults over 65 with various mental health issues including dementia. The post holder will be responsible for the assessment, formulation, implementation and evaluation of complex programmes of care and treatment specifically relating to residents within Inverclyde care homes.
When this care and treatment relates to distressed behaviours, they will use a strength based, recovery focused approach to care that proactively involves the person, their carers and other health and social care partners based on the principles of the Stress and Distress in Dementia approach.
- The post-holder is required to undertake duties without direct supervision as well as supporting and supervising work undertaken by registered and unregistered health and local authority staff.
- Promote partnership working with patients, carers and other professionals/agencies in line with legislative frameworks, agreed national and local standards and clinical policies.
- Practice will reflect a commitment to using best practice guidance to ensure the safe, effective delivery of person centred care.
- Provide support to Inverclyde OPMHT services (urgent response, Memory Clinic, care home and hospital liaison, community mental health nursing and care management) and maintain links with Alzheimer Scotland Post Diagnostic Link Workers.
- Work in a variety of home/near-to-home community environments and use current technology in line with remote working procedures.
- Participate in, and provide, regular caseload line management supervision and clinical supervision; undertake line management responsibilities as required.
- Actively participate in student nurse education via Practice Supervisor/Assessor responsibilities and support the ongoing learning and development of the staff team.
- Support the team lead in the ongoing learning and development of the OPMHT key functions.
- Live NMC UK pin or equivalent professional registration as required for the role.
- Experience of working with older adults with mental health issues, including dementia, and ability to work in a multi-disciplinary environment.
- Ability to undertake assessment, formulation and evaluation of care programmes; strong communication and collaboration skills.
- Experience in supervising staff and supporting student nurses is desirable.
