Job Title:

Shift:

Day Shift – 5x8-hour shifts

Marin County (up to 50-mile radius )

Job Description:

We are seeking a licensed Speech Language Pathologist (SLP) to provide home-based rehabilitation services to home health and hospice patients. The SLP will develop and implement individualized treatment plans, provide patient and family education, and collaborate with an interdisciplinary care team to ensure high-quality, coordinated care in the home setting.

Certification & Licensure Requirements:

• Active California Speech Language Pathologist License – Required

• BLS (Basic Life Support) – Required

Job Accountabilities

Rehabilitation Services:

• Develop and implement treatment plans for conditions including stuttering, delayed language, cognitive disorders, and swallowing disorders

• Complete patient evaluations and assessments in accordance with Speech Pathology Standards of Practice

• Develop, implement, and update patient Plans of Care (POC)

• Provide and progress therapeutic treatment per established plans and standards

• Complete all documentation related to treatment and charge capture per policy

• Coordinate care effectively with interdisciplinary team members and support staff

• Ensure compliance with departmental and organizational standards

• Maintain flexibility with scheduling, including days and hours as needed

• Assist with clinical supervision of assistants and aides

• Serve as clinical instructor for assigned students

Clerical Responsibilities:

• Prepare and maintain accurate, timely patient documentation in medical and departmental records

• Utilize Electronic Health Records (EHR) to retrieve, enter, and update patient information

• Perform general clerical duties as needed, including scheduling and communication support

Safety & Quality:

• Participate in quality improvement initiatives and interdisciplinary projects

• Maintain a clean, safe, and organized work environment

• Clean and disinfect equipment after each patient use

• Inspect and operate equipment according to manufacturer guidelines and report malfunctions

Compliance:

• Maintain strict confidentiality of all Protected Health Information (PHI)

• Deliver and document services in compliance with state and federal regulations

• Meet all departmental, organizational, accreditation, and licensure requirements

• Maintain certifications and complete required continuing education
