Schedule: 8:00-5:00 PM, Every 4th weekend on-call
Agency: Genesis Home Care
Territory: Brownsville, Fultonham, East Fultonham, Glenford, Gratitot, Rushville, Corning, Hemlock, Junction City
Responsibilities- Provide home-based physical therapy services as ordered by the attending physician.
- Interact with patients and their families, contributing to a compassionate, quality care experience.
- Home care context provides real-world perspective on helping patients restore health.
- Drive to patient locations per scheduled visits and maintain thorough documentation consistent with agency protocols and Medicare/Federal guidelines.
- Observe, record, and report patient reactions, changes, and progress to the attending physician and appropriate agency staff.
- Complete initial evaluation and admission visit within 48-72 hours of referral; develop and adjust the plan of care as needed.
- Promote continuity of care with appropriate admissions, transfers, and discharges.
- Educate patients and families on disease/injury management and proper use of equipment; involve patient and family in care planning.
- Supervise therapy services provided by PTAs and, when appropriate, nurse assistants; coordinate with schedulers and other team members.
- Complete clinical documentation in accordance with agency protocol and Medicare/Federal guidelines; perform chart audits as requested.
- Participate in on-call requirements, case conferences, orientation/training of new employees, and mandatory agency meetings.
- Physical Therapist with a current license in the state of employment.
- Minimum of one year of experience as a PT in an acute setting is required.
- Home care experience is desired.
- Ability to effectively communicate with patients, families, physicians, and co-workers.
- Knowledgeable on therapy best practices; ability to make appropriate physical therapy judgments.
- Detail-oriented, observant, and disciplined with the ability to prioritize and be timely.
- Valid driver’s license and auto insurance with your name as a listed driver.
- Drives to patient’s primary location per scheduled visit; daily attendance at assigned visit locations.
- Documents all aspects of visits within required time frames; adheres to plan of care within scope of practice.
Health Requirements:
Must be able to meet all physical position requirements. Evidence of a current negative tuberculin skin test or chest x-ray, or TB testing at orientation; annual TB testing thereafter. Employee must be free of conditions that would limit ability to perform job functions. May be at risk for exposure to blood borne pathogens. Travel required.
