Overview Coordinate Care. Support Teams. Improve Access After Hours.
We’re hiring a Remote After hours Coordinator to support our on‑call clinical team and ensure smooth transitions for patients needing home health, hospice, or palliative care services.
This telephonic, work‑from‑home position reports to the Manager of After hours Clinical Support and plays a vital role in supporting After Hours workflow, scheduling visits, referral coordination, and assisting on‑call nurses (RNs).
The shift is Friday 5pm-9pm and Saturday/Sunday 8am-8pm est.
What You’ll Do as an After hours Coordinator- Coordinate after hours patient care workflows and support on‑call RNs
- Schedule patient visits using HCHB (Homecare Homebase) system
- Review medical records using nationally recognized clinical criteria guidelines
- Process referrals and place patients in the appropriate care setting (home health, hospice, or palliative care)
- Educate patients and families about Gentiva’s services and next steps in the care journey
- Maintain accurate documentation and patient tracking
- Assist with admissions coordination by following up with branch teams
- Deliver exceptional customer service with compassion and professionalism
- Adhere to HIPAA, company policies, and compliance programs
- Participate in company‑wide training, projects, and performance initiatives
Specialized Knowledge/
Skills:
- Call center knowledge in a healthcare industry setting
- Ability to learn and master information related to locations and services of client.
- Outstanding computer skills.
- Ability to communicate effectively with empathy over the phone and while interacting with others; excellent interpersonal skills.
- Excellent customer service skills
- Outstanding verbal and interpersonal skills
- Good time management skills
- Ability to learn and master information related to locations and services of client
- Excellent analytical and problem‑solving skills.
Education/
Experience:
- Associate degree preferred
- Minimum of 1 year working in a healthcare related field or healthcare call center
- Excellent analytical and problem‑solving skills.
- Ability to learn and master information related to locations and services of client.
- Outstanding computer skills.
- Good time management skills
- Good working knowledge of home health, hospice and palliative care services
- Ability to communicate effectively with empathy over the phone and while interacting with others; excellent interpersonal skills.
Training/Equipment:
- Strong computer skills to include expert knowledge Microsoft Office Suite
- INTERNET REQUIREMENT:
High‑speed Internet connection (minimum 10 Mb/s download speed and minimum 2 Mb/s upload speed, recommended 5 Mb/s upload). Satellite based internet services are not acceptable. - Ability to learn new systems and technology
