Physical Therapist​/Occupational Therapist - Home Health

Smart Rehab is expanding and seeking Physical Therapists (PTs) and Occupational Therapists (OTs) to provide Medicare Part A home health therapy visits. This role offers full-time, part-time, or PRN caseload opportunities, consistent referrals, and a flexible schedule you control.

• Complete PT or OT evaluations, follow-up visits, and discharges in patients’ homes
• Deliver medically necessary home-based therapy under an established plan of care
• Monitor vital signs, safety, functional progress, and home environment risks
• Document visits using the provided EMR platform (training included)
• Provide caregiver education, fall prevention strategies, and safety recommendations
• Communicate with the interdisciplinary clinical team overseeing each case
• Full-time, part-time, or PRN volume available
• No productivity requirements

• Missouri licensed Physical Therapist or Occupational Therapist
• Experience with older adults preferred
• Home health experience helpful but not required
• Strong communication and documentation skills
• Comfortable working independently in home environments

• Independence without the burden of clinic productivity quotas

Smart Rehab has provided outpatient in-home therapy services to older adults since 2017. This role is part of Smart Rehab’s Medicare Part A division, which follows a traditional home health visit structure while maintaining our therapist-led, quality-focused approach.
