Overview
We are a large PCN of 11 practices with over 103,000 patients. This role is practice-based; applications for remote work are not considered. The PCN employs the majority of our Pharmacists and Technicians directly, enabling learning and development under the leadership of an experienced Lead Clinical Pharmacist.

Key Responsibilities- Working within the Primary Care Network team to undertake medication reviews, particularly in high risk groups such as:
- Frail elderly
- Poly-pharmacy
- Renal impairment
- Hepatic impairment
- Substance misuse
- Patients on high risk medicines
- STOPP/START identified patients
- Revolving door hospital admissions
- Improve prescribing practice through educational support for all prescribers within the Primary Care Network
- Lead on changes in prescribing across patient populations when evidence requires updates (e.g., drug withdrawals or changed indications)
- Liaise with community pharmacy colleagues to align support for medicines adherence
- Support improvements in clinical care through practice-based audits and implementation of changes
- Provide prescribing advice to prescribers in the PCN (e.g., temporary non-availability of drugs)
- Ensure patient safety when transferred between care providers through reconciliation of prescribed medicines
In addition to primary responsibilities, the clinical pharmacist may be requested to:
- Provide advice and support for patients with self-limiting conditions
- Encourage cost-effective prescribing within the PCN member practices
- Review medications for newly registered patients
- Implement and embed a robust repeat prescribing system and electronic repeat dispensing
- Re-authorise medication for repeat prescribing within scope of practice
- Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required
- Support clinicians with the management of patients suffering from drug and alcohol dependencies
- Actively signpost patients to the correct healthcare professional
- Assist with QOF targets, including running LTC review clinics (individual or group settings)
- Administer influenza and other required vaccines when requested
- Provide education on medicines for the clinical team
- Review the latest guidance to ensure the PCN conforms to NICE, CQC, etc.
- Act as the PCN’s point of contact for all medicines-related matters, establishing positive working relationships
Essential
- Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
- Minimum of 2 years post-graduate experience in pharmacy, with at least 1 year in general practice
- Member of the Royal Pharmaceutical Society (RPS)
Desirable
- Holds a prescribing qualification
- May hold or be working towards a postgraduate pharmacy qualification
- Awareness of the breadth of common acute and long-term conditions likely to be seen in general medical practice
- Member of or working towards (through foundation) RPS Faculty membership

- Holds a full driving licence and owns a car, as there is a requirement to travel between practices

- Knowledge of EMIS

- Relevant experience of service delivery within the NHS and PCNs
- Excellent influencing and relationship skills with colleagues
- Ability to work as part of a multi-disciplinary team
- Ability to engage effectively with a wide range of stakeholders including clinicians, patients and the public, ensuring effective two-way communication within the PCN member practices
Desirable
- Experience of supervising staff and empowering clinical professionals to improve cost effectiveness and quality of care
