Position: Clinical Pharmacist (Part-Time)

The Role

The Clinical Pharmacist works in conjunction with a multi-disciplinary team to provide safe, high quality infusion therapy to patients in the alternate site. This role is responsible for evaluating and monitoring drug therapy relative to the total needs/problems of the patient while collaborating with physicians, nurses, and other healthcare professionals in an effort to achieve the best possible outcomes.

Schedule: 2-3 days per week, Monday - Friday 8AM - 5PM.

Minimum Qualifications

Experience/Education:

  • Bachelor of Pharmacy
  • Active pharmacist license
  • Clinical Pharmacy experience from work or clinical rotation experience
  • Proficient at partner programs (i.e.: MS Outlook, Word, Excel)
  • Ability to cope with ambiguous, changing environment while under pressure in a calm manner
  • Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion team
Essential Functions
  • Reviews, receives, and verifies all patient orders and prescriptions and documents according to policy and procedure and state and federal pharmacy law
  • Participates in the direct supervision of Pharmacy Technicians, Pharmacy Support Technicians, and Pharmacy Logistics Technicians
  • Participates in on call rotation for after hours availability for either home infusion or ambulatory infusion
  • Maintains proper competencies based on assigned role
  • Maintains active licensure and current on all required continuing education
  • Acts as a drug information source for internal staff and outside referral sources
  • Provides comprehensive patient counseling to patients who accept and request a pharmacy consult
  • Reviews clinical information including applicable labs and patient information for the safe provision of medication
  • Adheres to all federal and state pharmacy law and regulations
  • Clarifies and receives orders from a physician or medical agent
