As a Physician Assistant or Nurse Practitioner, you’ll join a purpose-driven team that:
Responsibilities- Delivers continuum-based care – integrating primary, psych, palliative, and home-based services.

- Continuum-Based Care (Clinical Excellence):
Deliver high-quality care across home, senior living, and community settings, supported by a multidisciplinary team in palliative, psych, and neurology. - Dementia Leadership:
Build deep expertise in dementia-informed care and caregiver support as part of our expanding geri-psych and cognitive health initiative. - Autonomy with Support:

- Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
- Delivering dementia-informed, relationship-based care across the continuum of care.
- Conducting comprehensive assessments and developing tailored care plans for older adults.
- Managing chronic conditions and performing care with skill and empathy for patients and their families.
- Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.
- Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
- Maintaining consistency in documentation, follow-through, and patient communication.

- Active, unencumbered NP or PA license in the state you will be practicing.
- Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
- Compassionate, patient-centered approach and strong clinical judgment.
- Valid driver’s license and reliable transportation.

Pay Range for Posted Region: $140,000 USD - $200,000 USD
