Maintain and review personalised risk assessments, support plans, ensuring the active involvement of the young person to promote a person-centred approach.
Work closely with external agencies (e.g., social workers, healthcare professionals, police or ambulance service) and supporting young people to various appointments including health visits, college interviews, home office appointments or social worker meetings.
Identify development and training needs to promote employability, including help with education and college applications, whilst accurately recording and documenting this with feedback to the keyworker as required.
