• Conduct thorough evaluations of clients in their home environments and develop individualized therapy programs with measurable short- and long-term goals.
• Deliver treatments aligned with the physician's plan of care, documenting progress, reassessments, and discharge summaries in a timely and accurate manner.
• Act as a liaison with physicians, managed care providers, aides, and other members of the interdisciplinary team to ensure efficient coordination of care.
• Supervise a Physical Therapy Assistant (PTA): assess their ability to perform interventions, ensure required co-signatures, and monitor ongoing performance.
• Provide written home-exercise or care programs to patients/caregivers, give education based on their needs, and foster a respectful therapeutic relationship to promote goal achievement.
• Participate in discharge planning, respond promptly to voicemails/messages, update schedules in the system, and document communications across the care team.
• Meet productivity standards consistently, adhere to organizational competencies and standards of behavior, and carry out additional duties or projects as assigned.
• Physical demands: lift/push/pull at least 50 lbs and stand for a minimum of 8 hours per day.

• Bachelor's Degree in Physical Therapy or equivalent.
• Valid Physical Therapist license in the state.
• Minimum of six (6) months of physical therapy experience.
• Must be able to travel to 4-6 patient homes per day within Monmouth County in New Jersey
• Proficiency with PC/computer use and documentation systems.

• Doctorate in Physical Therapy (DPT).

• Skilled clinical evaluation and treatment planning in a home-care setting
• Strong verbal and written communication (patient, caregiver, and interdisciplinary team)
• Effective time-management and scheduling in a mobile/home-based role
