Pair Team is assembling a team of passionate individuals to transform primary care for those who need it most. We are seeking a full-time Bilingual Family Nurse Practitioner who can think creatively and empathetically to improve how people access healthcare. The role supports CalAIM’s Enhanced Care Management Medi-Cal benefit program, providing long-term, whole-person care coordination including behavioral health and social supports.

The successful candidate will engage directly with individuals living with Serious Mental Illness or Substance Use Disorder, those experiencing homelessness, and individuals with high medical needs. The Lead Care Manager collaborates with partner clinics, community organizations, and Pair Team’s Clinical Team to build trust and deliver holistic care.

• Provide best-in-class virtual preventive care for underserved patients using our care delivery technology and clinical judgment
• Develop and refine clinical programs to support patients holistically and address barriers in care
• Collaborate with our product team and provide feedback to improve our platform for the care team
• Work with a diverse and inclusive team

• 2+ years of clinical experience serving patients with complex social and healthcare needs; passion for building a more equitable healthcare system
• 1+ year of primary care experience
• Experience or desire to work with a multidisciplinary team
• Technology-savvy and experienced in digital-first healthcare (telemedicine) and comfortable with multiple tech platforms
• Board certification or eligibility in Family Medicine
• Comfortable serving patients across the lifespan (pediatrics to geriatrics)
• Experience working in Medicaid/FQHC clinics that are underfunded/underserved
• Bilingual, Spanish and English
• NP license in CA
• Active DEA license preferred
• Startup experience is a plus
• Remote-friendly with 100% remote work option and monthly stipend
