Key Responsibilities:
- Conduct thorough physical therapy evaluations in patients’ homes.
- Craft individualized treatment plans tailored to physician orders, assessment findings, and patient goals.
- Implement evidence-based therapeutic interventions to enhance strength, balance, mobility, and functional independence.
- Educate patients, caregivers, and families on home exercise programs, safety techniques, and assistive device usage.
- Monitor and document patient progress, treatment responses, and any changes in condition.
- Collaborate with physicians, nurses, and interdisciplinary team members to ensure coordinated care.
- Adhere to agency policies, state practice acts, and federal regulations.
- Maintain accurate clinical documentation as per agency and payer requirements.
- Contribute to discharge planning and recommend ongoing therapy or community resources as needed.
- Engage in case conferences, quality improvement activities, and continuous professional development.
Qualifications:
- Licensed Physical Therapist (PT) in Michigan.
- Current CPR certification.
- A minimum of 1 year of clinical experience, preferably in home health within the last 5 years.
- Valid driver’s license, reliable transportation, and proof of insurance.
- Strong clinical reasoning, communication, and time-management skills.
- A compassionate, patient-centered approach with the ability to work independently.
