Key Responsibilities:

• Conduct thorough physical therapy evaluations in patients’ homes.
• Craft individualized treatment plans tailored to physician orders, assessment findings, and patient goals.
• Implement evidence-based therapeutic interventions to enhance strength, balance, mobility, and functional independence.
• Educate patients, caregivers, and families on home exercise programs, safety techniques, and assistive device usage.
• Monitor and document patient progress, treatment responses, and any changes in condition.
• Collaborate with physicians, nurses, and interdisciplinary team members to ensure coordinated care.
• Adhere to agency policies, state practice acts, and federal regulations.
• Maintain accurate clinical documentation as per agency and payer requirements.
• Contribute to discharge planning and recommend ongoing therapy or community resources as needed.
• Engage in case conferences, quality improvement activities, and continuous professional development.

Qualifications:

• Licensed Physical Therapist (PT) in Michigan.
• Current CPR certification.
• A minimum of 1 year of clinical experience, preferably in home health within the last 5 years.
• Valid driver’s license, reliable transportation, and proof of insurance.
• Strong clinical reasoning, communication, and time-management skills.
• A compassionate, patient-centered approach with the ability to work independently.
