This is a fulltime teleworker opportunity in Oklahoma. Qualified candidates must reside Oklahoma and be willing to travel within the state.
The Housing and Community Support Services Specialist builds collaborative relationships with municipalities, counties, and state housing entities across the state of Oklahoma. They leverage county housing programs and initiatives and bridge growth and expansion of housing resource availability through grants and special housing planning initiatives. Works directly with Aetna Better Health of Oklahoma members and community stakeholders and agencies to address both emergent and urgent housing needs.
Incumbent will build strategic partnerships and collaborate with Oklahoma housing agencies and entities, to address housing instability in the Oklahoma Medicaid population.
- Develop policies and procedures for supporting member access to State permanent supportive housing.
- In partnership with the health plan and by utilizing State specific contract, establish a strategic plan to include objectives, required reporting, intended outcomes and results.
- Serve as health plan liaisons and cross collaborative partners with state agencies, provider groups, and community-based organizations.
- Provide training and education to both internal staff and external stakeholders.
- Support care management staff, when complex needs are identified, and assistance is needed with connection to services, additional resources, and transition planning.
- Attend health plan rounds which may include transitional rounds
- Partner, support and collaborate with State and local housing authorities.
- Provide support and expertise with determining housing program qualifications and eligibility; community development block grant programs; affordable housing and tenant support for individuals with disabilities.
- As the Aetna Better Health of Oklahoma Housing Subject Matter Expert, collaborate with the RFP team and assist with providing information for State RFP requests and renewals.
- Emphasize on reductions in care disparities and increased health equity.
- In partnership across multiple departments, coordinate the monitoring and tracking of outreach events which align and support community and membership goals.
- Proven track record meeting project dates.
- Broad knowledge of community resources including behavioral health and substance use disorder delivery system in Oklahoma
- Ability to interpret rules, regulations, and policies and to make decisions in accordance with established policies.
- Excellent verbal, written and presentation skills.
- Candidate must reside in Oklahoma.
* REQUIRED - 5+ years' experience in public sector or managed health care.
- 2+ years' experience in assisting the elderly and persons with disabilities to secure accessible, affordable housing through Federal (such as HUD, Shelter Plus Care, SAMHSA, and USDA), as well as, local programs.
- 2+ years demonstrated proficiency with personal computer, keyboard navigation, and MS Office Suite applications (Teams, Outlook, Word, Excel, SharePoint, etc.)
- Must possess reliable transportation and be willing and able to travel in-state up to 25% of the time. Mileage is reimbursed per our company expense reimbursement policy.
- Flexibility to work beyond core hours, Monday-Friday, 8am-5pm, to meet member needs.
- Experience in determining eligibility for programs or services or experience is preferred.
Bachelor's degree in Business Administration, Human Services or a relevant field is preferred, or an equivalent combination of education and experience.
