Overview
Sentara Health Plans is hiring for a Manager, Care Management/Registered Nurse (RN) for Maternity and High Risk Infants Team - Remote in VA.
Status:
Full-time, permanent position (40 hours)
Standard working hours: 8am to 5pm EST, M-F
Location:
Remote in Virginia only.
Responsible for managing and coordinating the patient care transitions program. This includes ensuring the patient's progression through the care continuum to achieve designed clinical and financial outcomes, participating in the system development and implementation of integration of care transitions, collaborating with multidisciplinary team across all settings (ambulatory, post‑acute providers and health plans), mentoring by role modeling professional practice consistent with organizational goals, and ensuring customer service, clinical effectiveness, and standards of care.
Also responsible for developing and managing the departmental budget and compliance with federal, state and regulatory requirements.
- Bachelor's Degree in Nursing (required)
- Master’s degree (preferred)
- Registered Nurse (RN) License (Compact or Virginia) (required)
- 1 year leadership experience (required)
- 3 years case management experience (required)
- Experience working with low and high‑risk pregnant population / Maternity / OB / L&D / Mother Baby / Postpartum (preferred)
