Patient Scheduler - Remote
- Must reside in the United States
Upstream Rehabilitation is looking for a Remote Patient Liaison Scheduler to join our team.
Job Description- Responsible for intake of information from referral source and scheduling of patients, including creation of an initial file.
- Obtain authorization, gather necessary information to complete registration, and arrange appointments at appropriate clinics for skilled therapy or functional testing.
- Support the mission, vision, and values of Upstream Rehabilitation; no supervisory duties.
- Complete tasks as directed to ensure patients receive timely scheduling that meets their medical needs.
- Review Raintree/Document routing and other sources to gather needed referral, records, and other information for scheduling.
- Use the Patient Permanent Selection system to schedule patients to the correct clinic/specialty.
- Obtain and record preferences for communication with case managers, networks, attorneys, or other entities.
- Respond to weekly reporting:
Scheduling Lag report, Referral Recapture Report, Missed Visit Report, contact metrics, Future Authorization report; report progress and issues to supervisor or leader. - Process FCEs and other functional testing as directed.
- Build and maintain relationships with clinics, referral sources, networks, and other partners.
- Remote contact center environment; required to work on phone 100% of the time.
- Other duties as assigned.
- High school diploma or equivalent required.
- Data entry experience.
- Ability to manage incoming phone lines.
- Relationship building, interpersonal skills, and multi‑tasking capability.
- Problem‑solving, diplomatic conflict resolution, and ability to handle difficult situations gracefully.
- Familiarity with Upstream products waiter: physical therapy, occupational therapy, dry needling, and other services.
- Proficient with computer‑based platforms including EMR, Microsoft Office Suite, tracking systems, and web‑based portals.
- Exceptional communication skills—oral, written, and presentations.
- Proficiency with Microsoft Office Suite or related software.
