We are hiring a licensed Occupational Therapist (OT) for a per diem / PRN home health position. This role is ideal for clinicians seeking flexible scheduling, per-visit pay, and the opportunity to deliver patient-centered occupational therapy services in the home setting.

• Perform occupational therapy evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide therapeutic activities, ADL training, and patient education
• Document care accurately and timely in compliance with HIPAA and clinical standards
• Communicate effectively with patients, families, and interdisciplinary care teams
• Deliver high-quality home health occupational therapy services

• Active Occupational Therapist (OT) license in good standing
• Graduate of an accredited occupational therapy program
• Ability to work independently and manage a flexible schedule
• Strong clinical documentation and communication skills
• Reliable transportation for in-home therapy visit
• Home health experience preferred, but not required
