Job Title: Customer Coordinator
Starting Salary: £27,693 progressing to £31,021 per annum
Hours: 37
Location: Stevenage with hybrid working

About The TeamThe Gateway Connections team is a hybrid office/home working telephone and virtual contact team all about working preventatively with adults who need information, advice, and support to stay independent in their own homes for longer. The multi‑disciplinary team works with Social Workers and Occupational Therapists and other professionals to achieve the best outcomes for people in Hertfordshire. We are a flexible team that promotes HCCs Ways of Working protocols.
AboutThe Role
You will promote and support a strength‑based approach to all interventions to maximise independence, health and well‑being by offering opportunities to prevent, delay or reduce the need for Adult Social Care. This will require you to apply your creativity working with residents to ensure their safety and continued independence. Your knowledge of local services to support people to access appropriate community resources is also crucial, as is working alongside our visiting CCOs, Social Workers and Occupational Therapists.
You will work successfully with partners and many other agencies for the benefit of citizens.
- Previous experience in a Social Care setting
- Ability to work autonomously as well as part of a team
- Previous experience communicating with vulnerable adults
- Previous experience in customer services involving vulnerable adults
- Empathic and patient when dealing with vulnerable adults
- Knowledge of Social Care legislation such as Care Act 2014 and Mental Capacity Act 2005
