Outreach Advocate (Career Path to Case Management)
Location:
Washington State (Applicants must reside in Washington to be considered). Full‑time, Hybrid/Remote during the Outreach phase. Future role requires 90% travel within your county of residence.
Hourly pay: $24 per hour.
This position begins as a full‑time Outreach Advocate for approximately six months, with a planned transition into a Case Manager role contingent upon performance, compliance, and business need.
Responsibilities – Outreach Advocate Phase- Manage referrals and maintain accurate, compliant, timely documentation.
- Conduct structured outreach and initiate timely contact with assigned referrals.
- Complete multiple contact attempts per referral in accordance with program standards.
- Support enrollment into the Health Home Program.
- Utilize geographic tools and system platforms to assign members to Care Coordinators.
- Accurately document all outreach activity in OneHAP and Lead Organization systems.
- Meet documentation quality, timeliness, and compliance standards.
- Maintain consistent communication with Care Coordinators, Leads, and internal departments.
- Support follow‑up, rescheduling, and member inquiries.
- Meet daily, weekly, and monthly productivity and outreach benchmarks.
- Participate in audits, coaching, and performance reviews.
- 90% travel within the county of residence.
- Home visits and community‑based member engagement.
- Develop and follow up on Health Action Plans.
- Complex care coordination and resource navigation.
- Continued performance metrics, audits, and compliance oversight.
- Must reside in Washington State.
- Must possess a valid driver’s license and active auto insurance.
- Associate’s degree preferred.
- Strong organizational skills and attention to detail.
- Comfortable working in a structured, metrics‑driven environment.
- Ability to juggle multiple platforms simultaneously.
- Proficiency in Microsoft Office (Word, Excel, Outlook, Teams).
- Ability to be on the phone for extended periods.
- Comfortable navigating multiple electronic systems.
- Willing to meet clearly defined productivity metrics.
- Ability to meet future travel requirements for the Case Management/Care Coordinator role.
- Call Center experience.
- Sales background and/or medical/healthcare background.
- Experience in customer engagement, outreach, or member services.
- Bilingual or bicultural candidates encouraged to apply.
- Clear performance expectations and measurable success metrics.
