LICSW or LMHC (HYBRID/REMOTE) – Community Health Connections
Community Health Connections is a Non‑Profit Federally Qualified Healthcare Center situated in Central Massachusetts. Opened in 2002, we serve underserved populations, providing compassionate care to everyone—regardless of income or insurance status. Under the supervision of the Director of Behavioral Health, the clinician will deliver a wide range of psychosocial evaluation, diagnostic, counseling, crisis intervention, and case‑management services in an integrated primary care setting.
Responsibilities- Provide counseling, therapy, and/or psychotherapy to clients and families; prepare treatment and discharge plans and follow‑up care.
- Collect and analyze patient data through interviews, case histories, psychological tests, and observational techniques.
- Consult with mental health and medical staff, make referrals, and arrange appropriate interventions for patients in crisis or with serious mental health/substance‑abuse issues.
- Refer clients to social service agencies for financial assistance and other services; may conduct home or community outreach.
- Track treatment effectiveness, adjust methods, and document progress for civil commitment hearings or legal proceedings.
- Maintain case files, referral records, and other documentation; stay updated on patient data.
- Collaborate with other legal and treatment agencies regarding patient records and rights.
- Plan and facilitate community outreach and education; act as liaison to community organizations and schools.
- Supervise lower‑level therapists and graduate students/interns; provide consulting services to other patient care professionals.
- Perform administrative tasks such as budget monitoring, report preparation, and correspondence; may engage in research projects and propose program policies.
- Deliver teaching, consultation, and in‑service training to relevant professionals.
- Carry out other related duties as required.
- Demonstrate commitment to the health center’s mission, values, and standards.
- Master’s degree in Counseling or Social Work.
- Licensed Independent Social Worker (LICSW) or equivalent; licensure pending is acceptable with temporary license documentation.
- Administrative decision‑making experience.
- Ability to observe, assess, and record symptoms and progress.
- Proficient in clinical social work and mental‑health counseling principles and procedures.
- Familiarity with psychological testing.
- Interviewing and psycho‑developmental evaluation skills.
- Skill in preparing and maintaining patient records.
Knowledge of community medical diagnostics and patient‑care services. - Knowledge of clinical operations and procedures.
- Clinical experience required.
- Awareness of community mental‑health resources and willingness to engage them.
- Ability to maintain emotional stability in emergencies and stressful situations.
- Multilingual skills (English/Spanish/Portuguese) are a plus.
- Strong interpersonal and communication skills; ability to work with diverse constituencies.
- Knowledge of crisis intervention techniques.
- Assessment and modification of therapeutic programs.
- Strong writing and editing abilities.
- Knowledge of social/vocational rehabilitation principles related to mental‑health practices.
- Knowledge of legal and ethical issues related to patients’ rights.
- Leadership and training skills for staff and students.
