Role SummaryThe Physical Therapist plans, organizes, and delivers comprehensive physical therapy services to support elderly and frail patients in maintaining independence, safety, and quality of life within the community. This role involves patient assessment, individualized treatment planning, hands-on therapy, and education of patients, caregivers, and families, while collaborating closely with a team to ensure coordinated, high-quality care across multiple settings.
Duties- Provide high-quality physical therapy services as an active member of the care team
- Perform in-person patient assessments at initial enrollment and as clinically indicated
- Evaluate the need for adaptive durable medical equipment, specialized footwear, prosthetics, and mobility aids
- Assess patient homes for safety and mobility; recommend appropriate home modifications and equipment
- Develop individualized physical therapy treatment plans in accordance with professional standards and organizational policies
- Administer physical therapy interventions to achieve established functional and rehabilitation goals
- Measure treatment effectiveness and revise care plans as needed
- Accurately and timely document evaluations, assessments, treatment plans, progress, and outcomes according to policy
- Educate patients and families on therapeutic procedures, home exercise programs, and safety strategies, ensuring understanding and proper documentation
- Train staff in body mechanics, safe transfers, gait techniques, equipment use, and strategies to maximize independence with activities of daily living (ADLs)
- Provide physical therapy services in centers, clinics, contracted facilities, and patient homes based on each patient’s plan of care
- Support patients in achieving the highest level of functional independence possible
- Maintain positive and professional relationships with coworkers, patients, families, and community partners
- Assist team members in understanding patients’ rehabilitation needs and functional goals
- Collaborate with the team to develop and implement comprehensive, patient-centered care plans
- Participate consistently in daily team meetings, communicating changes in patient status and contributing to coordinated care planning
- Perform duties of other team members as appropriate based on licensure, competencies, and experience
- Actively participate in quality improvement initiatives, organizational meetings, in-services, and community meetings
- Complete all required clinical documentation in patient medical records, including initial assessments, reassessments, changes in condition, and significant events
- Follow all policies, procedures, OSHA guidelines, and standard precautions
- Maintain confidentiality of patient, family, and employee information
- Maintain a safe work environment and comply with safety policies
- Participate in continuing education, required training, and maintain professional licensure, certifications, and affiliations
- Perform other duties as assigned
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current Physical Therapist license in the state of practice
- Valid driver’s license and proof of required auto insurance
- Two (2) years of experience working within a team-based environment in a hospital, nursing home, or community-based setting preferred
- One (1) year of experience working with a frail or elderly population preferred
- If prior elderly or frail population experience is not present, training may be provided upon hire (if applicable)
- Experience providing home-based or community-based physical therapy services
- Strong background in restorative and rehabilitative care, including pain management, safety interventions, and functional training
- Experience collaborating across clinical and social service disciplines
- Demonstrated commitment to a people-first, patient-centered approach
- Excellent verbal, written, and interpersonal communication skills
- Ability to work collaboratively within a team-based care model
- Flexibility and adaptability in managing changing patient and program needs
