Provider Practice Strategy & Ops Lead — Readmission Reduction

A leading healthcare organization is seeking a Strategy & Operations Lead in Provider Practice Management. This role will involve developing clinical solutions, identifying best practices, and driving operational improvements to reduce readmissions in collaboration with provider groups. The ideal candidate will have at least 7 years' experience in population health strategy and an understanding of transitional care management.
