Beeston Primary Care Network (PCN) have a vacancy for a highly motivated and enthusiastic Clinical Pharmacist to come onboard. The team is extremely well integrated into the PCN and works very closely with Middletonand Hunslet PCN having a joint Executive Team across both PCNs. Their work is highly valued and recognised byall member practices. The PCN roles are well supported by the Clinical Director.

You will be working towards the delivery of the outcomesoutlined in the Primary Care Network Contract Enhanced Service Specification.

The post holder will be employed by South East Leeds GP Group, but will be deployed and managed day to day from within the Middleton & Hunslet Primary Care Network

There will be an element of office / home working, but it is amainly patient facing role. This is an exciting opportunity to use and develop your clinical knowledge and skills to contribute to all aspects of medicines optimisation and improve the lives of patients across the PCN.

Applications from all pharmacy sectors are welcome however, experience and knowledge ofworking in primary care would be desirable.

All PCN pharmacy staff are required to enrol on (or be exempted from) the 18-month CPPE Primary Care Pharmacy Education Pathway. You will beallocated a clinical supervisor and receive support from the Lead Pharmacist to complete the training and gain confidence in the role.

About us

Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some yearsfor a range of reasons, including improving the ability of practices to recruitand retain staff; to manage financial and estates pressures; to provide a widerrange of services to patients and to integrate with the wider health and care system more easily.

PCNs were formally ratified in July 2019.

Middleton and Hunslet PCN is a large PCN with a patient list size of circa 33,000 patients.

Of this, we currently provide clinical support to 10 Care Homes, with apopulation of around 360 beds. We have a significant workforce in place whichbrings wonderful opportunities for in-house multi-disciplinary team working. Wehave a well-developed multidisciplinary team including a Pharmacy Team,Occupational Therapists, Social Prescribers, a range of nursing/ HCA staff andmore which means that the majority of any patient's needs can now be provided on their own doorstep.

Middleton & Hunslet PCN also workscollaboratively with Beeston PCN, forming BMH (Beeston, Middleton & Hunslet

PCNs, we have roles working across both ensuring a continuity of care between.

To note this role is for that only of Middleton and Hunslet PCN.

Job responsibilities

Main Tasks:

Clinical

To improve the outcomes for patients fromtheir medicines and reduce the risk of harm. Part of this will involveimproving medicines safety and medicines management processes across the PCNsuch as contributing to actioning MHRA alerts and reconciling of medicinesupon transfer of care.

To carry out structured medication reviews(SMRs) to optimise medication for complex patients, in particular the frail,polypharmacy and care home residents. This may involve home visits for thehousebound which can be done jointly with the PCN care co-ordinators in thefrailty team. We have a structure in place for working smoothly with the manycare homes across Beeston.

Contribute to delivering the medicines aspectsof the investment and impact fund (IIF) which forms part of the PCN directedenhanced service (DES).

Join established MDTs locally to deliverproactive, personalised and coordinated health and social care. Partnershipworking across all care sectors and effective communication will be required.

Technical & Administrative

Support the PCN multi-disciplinary team toensure efficient medicines optimisation processes are being followed.

Implement efficient ordering and returnprocesses and reducing medication wastage.

Provide training and support on the legal,safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).

Promotion of Electronic Repeat Dispensing(eRD) and online ordering

Develop relationships with other pharmacyprofessionals and members of the multi-disciplinary team to supportintegration across health and social care including primary care, communitypharmacy, secondary care and mental health.

Support practice reception teams in streaminggeneral prescription requests, to allow GPs and clinical pharmacists toreview the more clinically complex requests.

Support the implementation of nationalprescribing policies and guidance within GP practices, care homes and other primary care settings.

Support the PCN to deliver on the Quality Improvement Plans, QOF and locally commissioned enhanced services

Support the PCN in reviewing and developingpractice policies for CQC requirements
