Plans, directs and coordinates the operational, financial and personnel activities of Inpatient hospice services, Outpatient Hospice services and alignment of hospice services with the Palliative Care program. Responsible for management of Hospice programs within parameters to meet State and Federal regulations for a hospice inpatient Unit and home based services. Accountable for achieving the key strategies and deliverables of the hospice and Palliative care programs.
Positions Hospice as integral to PHS continuum of care services and as a market leader.
- Responsible for program planning, development and operation as well as positioning program for future success and growth. Assures implementation of change projects/initiatives. Assists in the development and implementation of the department strategic plan. Deploys meets or exceeds requirements or benchmarks for quality clinical care, staff performance and development, customer loyalty, financial performance and growth within the department.
- Develops and deploys department work plans and goals or deliverables to support PHHs and PHSs key strategies and initiatives.
- Prepares, implements and monitors department operating and capital budgets in conjunction with overall PHH plan and strategies.
- Responsible for execution and coordination of all contractual agreements for hospice, i.e. pharmacy, DME, providers etc.
- Assures coordination of services with Palliative Care and other PHH and PHS services.
- Performs personnel management functions including hiring, coaching, evaluating, counseling and dismissing. Holds staff and managers measurably accountable for clinical outcomes, productivity and efficiency and customer loyalty goals that sustain departmental success.
- Develops, implements, and monitors standards based practice and assures quality care is delivered to all patients. Ensures completion of all required documentation for Hospice and Palliative patient care, ensures that the plans of care are developed in conjunction with the patient, patient's family and/or patient's representative and reflect the appropriate level of care.
- Oversees audits and continuous improvement of results as indicated.
- Develops and maintains policies and procedures for the Hospice and Palliative care programs as required.
- Maintain adequate knowledge of all functions of the department and maintain availability to assist with any problems that arise, whether routine or systemic.
- Rotates administrative on-call to provide general supervision during non-office hours.
- Plans, schedules, coordinates and attends regularly scheduled Interdisciplinary Team Meetings with Inpatient and Outpatient Hospice management and care delivery staff.
- Interacts and coordinates activities with a variety of health care agents, regulatory agencies, and vendors.
- Facilitates effective communication between administration, staff, physicians, other departments, patients, families, and the general public.
- Participates in conflict resolution as necessary and acts as an effective problem solver.
- Empowers professional staff through delegation of responsibility, authority, and accountability.
- Utilizes outcome based decision making (PDSA) for change initiatives.
- Performs other functions as required.
Masters degree in a health care related field required. 7 years home health/hospice care administration/management required. 1-3 years Hospice experience preferred. 1-3 years supervisory experience preferred. 3-5 years of direct patient care preferred.
