Key Responsibilities- Lead on the safe, timely, and effective discharge of patients from hospital settings
- Provide senior-level oversight and guidance on complex cases
- Work closely with patients, families, and multidisciplinary professionals to coordinate post-discharge support
- Develop, review, and implement robust care and discharge plans tailored to individual needs
- Act as a key liaison between hospital teams and external agencies to ensure continuity of care
- Proactively identify and resolve barriers to discharge, ensuring smooth transitions into the community

- A recognised Social Work qualification
- Current registration with Social Work England (SWE)
- Experience with in hospital discharge and complex case management (senior experience highly desirable)
