Key Responsibilities
  • Lead on the safe, timely, and effective discharge of patients from hospital settings
  • Provide senior-level oversight and guidance on complex cases
  • Work closely with patients, families, and multidisciplinary professionals to coordinate post-discharge support
  • Develop, review, and implement robust care and discharge plans tailored to individual needs
  • Act as a key liaison between hospital teams and external agencies to ensure continuity of care
  • Proactively identify and resolve barriers to discharge, ensuring smooth transitions into the community

Requirements
  • A recognised Social Work qualification
  • Current registration with Social Work England (SWE)
  • Experience with in hospital discharge and complex case management (senior experience highly desirable)
