Field Advance Practice Palliative Care Provider (NP or PA)

Hampton, Newport News, VA and surrounding areas
The Field Advance Practice Palliative Care Provider is a field-based role enabling clinicians to provide palliative care in patients' homes, board and care facilities, assisted living and independent living communities, with occasional office attendance for meetings or training. The role may include virtual care to support patients remotely. Per policy on hybrid/virtual work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered unless an accommodation is granted as required by law.
The Field Advance Practice Palliative Care Provider collaborates with company physicians, the patient’s other providers, and family members to develop complex plans of care aligned with the patient’s health status and goals. The provider offers clinical and non-clinical support to palliative care patients within a designated territory.
How you will make an impact:
- Provides palliative care services in home settings to patients.
- Develops and implements clinical plans of care for adults with chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic illnesses).
- Gathers history and performs physical exams and diagnostics as needed, then develops and implements treatment plans aligned with patient goals of care and current conditions.
- Meets patients’ and families’ physical and psychosocial needs with support from the interdisciplinary team.
- Educates patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention.
- Maintains contact with other clinical team members and the patient’s other physicians to coordinate optimal care and resources in a timely manner and in compliance with state regulations and company standards.
- Maintains patient medical records and documentation consistent with state regulations and company standards using Epic EMR.
- Participates in continuing education as required by state.
- Presents and prescribes medications as permitted by state authority.
Minimum Requirements:
- MS in Nursing or MPAS or MSPAS.
- Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in Virginia.
- DEA registration or eligibility required.
- CPR certification required.
- For Carelon Health business unit, satisfactory completion of a Tuberculosis test is a requirement for this position.
- Travel may be required.
Preferred Skills, Capabilities and Experiences :
- MPAS or MSPAS is highly preferred if a PA.
- 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care preferred.
- Experience in bedside hospital, oncology, emergency room (ER) or intensive care unit (ICU) settings preferred.
- Experience with Electronic Medical Records (EMR – Epic) preferred.
- Bilingual in English/Spanish preferred.
