OFFERED SALARY RANGE: $21.65-$23.42 per hour.
FULL SALARY RANGE: $21.65 - $57.73 per/hr.

The full salary range representing the broader compensation band for the position and progression within that range is based on employee longevity and continued service. SummaryWe’re pleased to announce an opportunity for the position of Health Home Care Coordinator within the Community Based Teams Department.
The Health Home Care Coordinator provides comprehensive care coordination services to eligible individuals and their families. This role involves assessing member needs, developing and monitoring individualized service plans, making appropriate referrals, and advocating on behalf of members with other service providers. Care Coordinators maintain a dedicated caseload and ensure consistent monthly engagement with assigned members across various settings. Health Home Care Coordinator’s support members in identifying and accessing resources, delivering health education, and applying motivational interviewing techniques to foster goal achievement, resilience, and healthy lifestyle choices.
The Care Coordinator promotes wellness through coaching and awareness of chronic health conditions, aiming to reduce emergency service usage and prevent hospital readmissions.
