The Family and Children’s Medicaid RN Care Coordinator serves as a clinical resource to the other care team members, helps the member obtain medically necessary services and collaborates with the behavioral health Care Manager and Community Navigator and the member’s providers to coordinate the delivery of the member’s social, behavioral, and medical services and supports.

Primary Responsibilities
  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
  • Outreach to members to complete a Comprehensive assessment to identify gaps in care and any barriers
  • Identify the most appropriate resources and or support systems that will be included as part of the member’s multidisciplinary team
  • Use evidence-based clinical policies and guidelines as the framework for clinical decisions and service provision that optimizes member care and outcomes
  • Outreach regularly to member to assess closure of identified gaps
  • Focus on the reduction of avoidable unplanned hospital admissions, impactable medical expense and reducing the burden of illness for members with complex or acute medical and behavioral conditions
  • Identify and engage members at-risk for readmission after discharge from an inpatient stay, focus on minimizing hospital and ER visits
  • Assist with transitions of care and provide outreach and follow-up to members following hospital discharge
  • Assess for any barriers to closing gaps in care and develop a strategy to overcome barriers
  • Create care plans using the appropriate OGI’s (opportunity, goals, and interventions)
  • Refer members to appropriate community resources and/or programs and provide timely follow-up
  • Improve member care, service, experience, access through end-to-end case management
  • Engage and communicate with the multidisciplinary team members on an ad hoc and ongoing basis to document and track their participation in the member record
  • Communicate with the PCP, specialists, and other care providers to coordinate care needs outlined in the care plan
  • Perform accurate and timely documentation in the electronic medical records
  • Serves as a clinical resource to the other care team members
  • Comply with all departmental policies and procedures

Required Qualifications
  • Current, unrestricted RN license in the state of MN
  • 5+ years of Care Management experience
  • 2+ years of experience with high-risk pregnancy, NICU, pediatric members
  • Experience working with the needs of vulnerable populations
  • Familiarity with the resources available in the community
  • Knowledge of Medicaid population

Proficient in using computer based charting and similar programs
