Overview
As a Home-based Clinician with CNS Healthcare, you will provide in-home therapy, crisis intervention, and case management to children with serious emotional disturbance and their families. You will work with a strength-based approach, emphasizing assertive intervention, parent and professional teamwork, and community involvement with other service providers. You will conduct in-home family and individual therapy, behavioral training and intervention, and ensure case management services are delivered according to the individual plan of service (IPOS).
EssentialFunctions
- Provide individual therapy, family therapy, group therapy, and case management in alignment with IPOS using evidence-based practices and a trauma-informed approach.
- Perform comprehensive assessments, diagnosis, and evaluation as clinically appropriate.
- Complete appropriate screening and other assessments as clinically appropriate.
- Improve therapeutic supports and functional skills in education, employment, social supports, wellness, self-management, and prevention of out-of-home placement.
- Provide crisis intervention when needed. Complete lethality risk assessments with dispositions and IPOS recommendations.
- Collaborate with the case holder to support person-centered planning and identify needs and interventions.
- Coordinate care with internal and external providers for assigned cases.
- Manage caseloads, ensure documentation and authorizations are up-to-date, and follow admission, transition, and discharge procedures.
- Facilitate intake of new persons served as assigned.
- Participate in team meetings, focus groups, work groups, and committees as assigned.
- Support agency staff and community partners by providing training as assigned.
- May participate in internal and external presentations about home-based services and may provide transportation for persons served.
- Meet direct service productivity and documentation standards per agency and program expectations.
- Maintain compliance with quality initiatives including MU, MIPS, CARF, and CCBHC requirements.
- Ensure CNS Healthcare service standards are applied in interactions with individuals served, guests, and staff.
- Engage in professional growth through ongoing education and conferences.
- Perform other related duties as assigned.
Required
- Master’s Degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy, AND
- State licensure: LLMSW, LMSW, LLPC, LPC, LLMFT, LMFT, TLLP, LLP, or LP
- Child Mental Health Professional designation within 90 days of hire and maintain thereafter; at least 1 year of experience with minors and their families
- Proficient with computers, Microsoft Office, and electronic health records (EHRs)
- Ability to contribute to an inclusive environment and work with individuals with mental or physical limitations
- Effective communication, collaboration, and relationship-building skills
- Discretion and judgment for confidential matters
- Valid and unrestricted Michigan Driver’s License and reliable transportation with proof of auto insurance
- Intellectual and Developmental Disability Professional (QIDP) designation within 90 days; 1 year of experience with individuals with intellectual or developmental disabilities
- Experience with children across age groups, solution-focused brief therapy, substance use treatment, and motivational interviewing
- Experience in community mental health settings; 1 year of clinical experience in individual and group psychotherapy under supervision; 1 year of experience evaluating children with severe emotional disturbances
The compensation range varies by licensure status; starting pay is $57,600 to $74,000 for limited licensed candidates and $61,850 to $80,150 for fully licensed candidates, based on non-discriminatory factors such as skills and experience.
