Medication Reconciliation Pharmacy Technician

Overview

The Medication Reconciliation Pharmacy Technician is a certified pharmacy technician that will function primarily within the medication reconciliation process. The Technician will review the medication list of newly admitted patients, make changes to the medication profile including but not limited to clarifications of dose, dosage form, and medication directions. An accurate list of changes will be presented to the Medication Reconciliation Pharmacist for review.

• Perform a patient medication history; document the history accurately and completely. Actively include the patient or the patient’s family, patient’s pharmacy, patient’s physicians, patient’s prescription bottles to ensure proper identification of doses and medications, including OTCs, herbals/supplements, and/or query software programs to resolve all discrepancies in a timely manner.
• Determine medications that may be needed to be brought from home based on formulary status, therapeutic interchanges and availability of medications. Non-formulary medications will be brought to the attention of the Pharmacist.
• Compile and clarify medication-related issues in the medical record so that the attending physician and/or designee can review and reconcile the list in the appropriate software.
• Present a complete and accurate admission list to the Medication Reconciliation Pharmacist for review. Prioritize patients at highest risk for readmission and those with polypharmacy.
• Document, review and submit appropriate Performance Improvement metrics to the Leadership Team using the current Electronic Health Record.
• All other duties as assigned within the scope and range of job responsibilities.

• Education:
  
  High School Diploma or GED.
• Credential(s): CPHT within 12 months of hire, if not already obtained.
• Experience:
  
  Required:
  
  One year of experience as a pharmacy technician or equivalent (e.g., Pharmacy School student). Preferred:
  Two years as a pharmacy technician.

Shift: Variable

Base pay range: $18.73/hr - $28.09/hr
