⭐ Education & LicensureMaster’s Degree in Social Work (MSW) required
Vermont LMSW required;
LICSW preferredAt least 2 years of relevant experience (healthcare experience preferred)
Strong communication, counseling, and rapport-building skills
Ability to assess psychosocial needs and explain medical information clearly
Experience with crisis intervention, community resources, and care management
Strong knowledge of local community resources
Knowledge of Medicare/Medicaid, insurance processes, and mandated reporting
Reliable transportation and valid driver’s license (travel required)
Ability to work collaboratively with patients, families, and interdisciplinary teams
Complete comprehensive in-home psychosocial assessments
Support patients and families in coping with illness and disability
Coordinate care plans with medical team and community partners
Provide crisis intervention, short-term counseling, and education
Document assessments and care plans according to regulatory guidelines
Participate in team meetings, care conferences, and quality improvement activities
Work in patient homes, community settings, and varied environments
Occasional exposure to emotional situations or environmental stressors
