The role involves creating connections between diverse, underserved, and vulnerable populations and Strive Health’s interdisciplinary care model. Building trust and promoting engagement are key objectives.

• Meet or exceed daily outreach expectations in phone calls, connections, and patient engagements.
• Proactively outreach current and prospective patients via phone to educate and enroll them.
• Use creative strategies and campaigns to engage patients in Strive’s care model with empathy.
• Conduct face‑to‑face outreach, including at‑home door‑knocking or at clinic visits.
• Liaise between multiple service providers, facilitating enrollment in services and community resources with culturally competent care.
• Utilize knowledge of local resources to manage Social Determinant of Health needs.
• Administer health screening assessments (HRAs) to complete patient enrollment.
• Review patient EMRs to identify potential barriers to care and unmet SDoH needs.
• Build rapport quickly with patients and external providers.
• Identify and perform mandatory reporting as required by state regulations.
• Perform other duties as assigned.

• 2+ years combined education, experience, or certification in the community health space.
• Community Health Worker Certification or equivalent required.
• 1+ year experience enrolling patients or customers into a health or care program, or promoting/selling services to end users.
• Reliable transportation, valid driver’s license, and ability to travel across assigned region.
• Experience with phone outreach.
• Internet connectivity – minimum speeds 3.8 Mbps (down) / 3.0 Mbps (up).

• Experience working in a multicultural setting.
• Experience with patients with complex medical needs.
• Experience with in a managed care or Medicaid plan.
• Experience with kidney patients.
• Experience with translation lines and services.
• Basic computer skills.

• Strong communication, organizational, critical thinking, and problem‑solving skills.
• Motivated, outgoing, attentive to detail.
• Extensive knowledge of community resources.
