We are looking for a Primary Care Liaison Practitioner to work within PCLS teams, the main integrated referral point into adult secondary mental health services. The role will provide mental health support, brief interventions, advice and signposting for service users, carers, GPs, Health and Social Care and third sector.
The multi‑disciplinary team includes nurses, psychologists, psychiatrists, non‑medical prescribers, and social workers. You will be a team player, sensitive and tactful, with excellent communication and interpersonal skills.
Responsibilities
- Provide comprehensive assessments and psychoeducation to individuals referred through a range of sources.
- Screen and triage referrals and participate in multidisciplinary discussions to support decision making.
- Deliver brief interventions, manage treatment plans and contribute to service development.
- Utilise video‑conferencing platforms for remote assessments and interventions where appropriate.
Qualifications and Experience
- Relevant professional registration or degree level education, or equivalent knowledge.
- Experience working in adult mental health services or a related multidisciplinary setting preferred.
- Knowledge of community referrals and brief interventions.
- Evidence of strong communication, organisational and analytical skills.

Flexible working, compressed hours and a blended approach to home working will be considered to meet service needs.
Note:
This post is open to newly qualified social workers. All newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role band, and are expected to complete their ASYE within 2 years of employment (with possible extension).
If appointed to an advertised Band 6 role, automatic progression follows successful ASYE completion. If appointed to a Band 5 post, eligibility for subsequent Band 6 vacancy upon successful ASYE applies.
Working hours:
Monday to Friday, 8am‑6pm offering assessments for adults aged 18 years and older. The team provides a duty function that includes referral screening and triage.
