We’re looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. RN: $30.00 per hour.

The RN provides skilled visits in patients’ homes. This position will be serving Trumbull County.

• Administer medication in accordance with standards of practice.
• Encourage social and habilitative skills and encourage self‑care and independence.
• Perform client treatments in accordance with Plan of Care.
• Assist the consumer in learning appropriate self‑care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor.
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care.
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the standards of the agency.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in‑services, orientation and staff meetings.
• Provide service in accordance with agency’s philosophy, standards, policies and scope of practice. Participate in Performance Improvement and adhere to HIPAA guidelines.
• Other duties as assigned.

• Valid State RN License.
• Valid driver’s license and use of automobile required due to travel involved.
• Annual MUI (Incident) training as dictated by State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.
• Ability to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
• Computer literacy (computer, laptop or tablet).
• Independent thinking, problem recognition, information gathering, and knowing when to contact a supervisor.
• Excellent customer service and exceptional client care while adhering to agency and program rules.
• Oasis experience required.
