Base pay range$21.00/hr - $23.00/hr
Job DescriptionThe Youth & Family Specialist (YFS) provides comprehensive support to children and their families through in-home and community-based interventions, while also providing education to address the client’s therapeutic goals.
The services provided in this role may include parent education, living skills training, behavior management training, behavioral observations in various settings, consultations with school personnel, and social skills training.
Responsibilities- Provide ongoing case management/care coordination services to clients and families.
- Serve as facilitator of Child & Family Team (CFT) meetings and complete all documentation.
- Facilitate psych-educational groups, as required.
- Perform case management services for children with serious behavioral health issues and their families.
- Assist clients and families in obtaining community support, identifying resources and resolving barriers.
- Complete all appropriate documentation (contact notes, BHSP, SN&CD, Crisis Plans, DSP referrals, SCA, ASR, etc.).
- Work effectively within the System of Care Model.
- Collaborate with CFT, medical, treatment, and management teams.
- Act in accordance with current ethical standards and professional boundaries.
- Perform other related job tasks as assigned.
- May conduct assessments, screenings, and intakes for the clinic, if properly credentialed and privileged.
- Bachelor’s Degree in a behavioral health-related field; or a Bachelor’s Degree in any discipline with at least one year full-time direct behavioral health work experience; or an Associate’s Degree with two years of experience; or a high school diploma with 4 years of case management experience.
- Experience in behavioral health; inpatient/residential; or group home/DCC/CPS in-home work experience.
- Ability to adjust to changing priorities and duties.
- Ability to work a 40-hour flexible schedule, including some nights and weekends.
- Strong verbal and written communication skills.
- AZ DPS Level 1 Fingerprint Clearance Card.
- AZ Driver’s License and valid auto insurance meeting organization requirements.
- Bilingual or bicultural background is a plus.
