Integration Specialist (Direct Care) at Nonotuck Resource Associates, Inc.

Nonotuck Resource Associates is actively seeking a dedicated Integration Specialist to join our team and provide community and home services, along with direct care supports to individuals with developmental disabilities enrolled in our Community-Based Day Supports program. This is a role focused on fostering independence, creating meaningful connections, and supporting individuals toward a full community-based life. Hours are split between our Brooklyn St.

Project in North Adams and our Community-Based Day Services in Pittsfield, Lee, and Adams. This is a full-time, non-exempt position with a hybrid office/work-from-home model.

• Individualized Support:
  Develop a deep understanding of each person supported, including their strengths, needs, goals, and desires; assist individuals in articulating their needs to facilitate personalized care.

  Work as a member of a team to support individuals in developing a full community-based daytime life; collaborate with Program Director, Assistant Program Directors, Shared Living Providers, and other stakeholders.
• Positive Image and Relationships:
  Ensure each person presents a positive image of themselves and the program; assist in developing long-lasting and meaningful relationships with family, friends, community members, co-workers (where applicable), and active community participation.
• Community Engagement:
  Support participation in community activities, including group memberships, volunteer commitments, educational opportunities, leisure and social pursuits, and employment where applicable.
• Documentation and Monitoring:
  Complete Individual Service Plan goals and activities in collaboration with the team; monitor activities of daily living and provide necessary support.
• Communication:
  Work closely with guardians, family members, community health care practitioners, and other institutions to ensure comprehensive and coordinated care.

• 2 years of experience working with individuals with developmental disabilities (preferred).
• High School Diploma; computer and organizational planning skills.
• Strong interpersonal and communication skills; ability to work collaboratively in a team environment.
• Flexibility and adaptability to meet changing needs of individuals supported.
• Experience with Department of Developmental Services, Adult Foster Care Services, HCBS waivers (supporting those with brain injury or chronic conditions) (preferred).
• Background check required.
• Valid driver’s license with an excellent RMV record (subject to RMV background check).
