Are you a social worker skilled who wants to use your clinical expertise to build something meaningful? At Seasons Adult Foster Care, our Program Director doesn't just manage compliance, you'll shape the quality of life for individuals with persistent chronic mental illness while leading a team that genuinely cares about the people we serve.

This isn't traditional social work. It's clinical leadership with impact.

As our clinical authority, you'll provide case management for up to 26 clients while leading House Leads and Designated Coordinators to deliver person-centered behavioral health services. You'll develop service plans, coordinate complex care, ensure Rule 245D compliance, and mentor staff to provide exceptional therapeutic support.

• Leading clinical decision-making for individuals with complex psychiatric and medical needs
• Building collaborative relationships with psychiatrists, county case managers, and healthcare providers
• Conducting chart audits and driving quality improvement initiatives
• Training staff on behavioral interventions, crisis management, and person-centered approaches
• Serving as the go-to expert when clinical situations get complicated

You will work Monday-Friday, on-site in our Staples office with travel to our program homes and are also part of our on-call rotation.

• Active Minnesota LSW license
• BSW or a healthcare-related bachelor’s degree
• CCM or ACM certification (or willingness to obtain within 12 months)
• 2+ years supervisory/management experience in behavioral health or human services
• Clinical social work experience with individuals with serious mental illness
• Deep understanding of Minnesota Rule 245D

Base pay range: $70,000.00/yr - $90,000.00/yr. Actual pay will be based on your skills and experience.
