- Provide consistent treatment updates to referring providers and answer questions about referred clients
- Develop comprehensive discharge plans for clients completing treatment
- Make accurate referrals to aftercare resources and follow up to verify placements
- Collaborate with internal teams to ensure all necessary information is communicated effectively
- Bachelor’s degree in health sciences, communications, psychology, social work, or a related field
- Minimum of 2 years of relevant work experience in healthcare, preferably in customer or patient-facing roles
- Strong interpersonal and relationship-building skills
- Excellent written and verbal communication abilities
- Proficient in Salesforce and Google Suite/MS Office
- Experience in case management, discharge planning, referral relations, or admissions
- Metrics-oriented mindset with experience working against concrete targets
- Ability to thrive in a fast-paced environment and learn quickly
