We are seeking a skilled and compassionate PRN Speech Therapist (SLP) to provide in-home speech therapy services to adult and geriatric patients. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact in patients’ lives.

• Perform in-home speech, language, cognitive, and swallowing evaluations
• Develop and implement individualized treatment plans per physician orders
• Provide skilled speech therapy interventions
• Educate patients and caregivers on treatment techniques and safety
  
  Monitor progress and adjust care plans as needed
• Complete accurate and timely clinical documentation
• Collaborate with interdisciplinary care teams
• Maintain compliance with HIPAA, Medicare, and agency standards

• Graduate of an accredited Speech-Language Pathology program
• Active Virginia SLP license required
• Minimum 2 years of clinical experience preferred (home health or SNF a plus)
• Strong skills with adult and geriatric populations
• EMR experience required (Kinnser/Well Sky or Homecare Homebase preferred)
• OASIS experience preferred
• Valid driver’s license and reliable transportation
