Position Overview

We are seeking dedicated and compassionate Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (OTAs) to join our expanding home health team.

In this role, you will help patients maximize independence in daily living through individualized, functional, and evidence-based interventions delivered in their homes. OTs will complete evaluations and develop plans of care; OTAs will implement therapy programs under OT supervision.

We offer flexible scheduling, assignments close to home, and opportunities to define your own coverage areas based on your availability.

Location

This position serves patients across many communities throughout Massachusetts. OTs and OTAs from any region of the state are encouraged to apply.

Key Responsibilities For Occupational Therapists (OTs)
  • Perform initial and ongoing assessments of functional status, ADLs, IADLs, cognition, safety, and home environment (including OASIS, where applicable).
  • Develop, implement, and revise occupational therapy plans of care that address patient goals, safety needs, and physician orders.
  • Identify deficits in functional mobility, strength, coordination, cognition, and sensory processing that impact daily living.
  • Educate patients and caregivers on therapeutic techniques, compensatory strategies, safety training, fall prevention, and home modifications.
  • Recommend, train, and assist in acquiring adaptive equipment and durable medical equipment (DME).
  • Provide supervision and clinical guidance to OTAs in accordance with state and agency requirements.
  • Communicate updates to the physician at least every 30 days or when changes occur, obtaining new or revised orders as needed.
  • Complete documentation, including evaluations, progress notes, and discharge summaries within agency timelines.
For Certified Occupational Therapy Assistants (OTAs)
  • Implement the OT-established plan of care through purposeful and functional interventions.
  • Provide ADL/IADL training, therapeutic exercises, cognitive strategies, energy conservation techniques, and safety education.
  • Observe and report patient progress, concerns, or changes in condition to the supervising OT.
  • Document treatment notes per visit in accordance with agency and regulatory requirements.
  • Follow supervision schedules as required by state regulation and agency policy.
Shared Responsibilities (OT & OTA)
  • Partner with the interdisciplinary team (Nursing, PT, Speech, HHAs) to support coordinated, patient-centered care.
  • Participate in case conferences and in-service training for ongoing professional development.
  • Assist with discharge planning when therapy goals are met or services are discontinued.
Admission Support (OT only)
  • Participate in the admission process when OT services are ordered, including assessment of home safety, functional level, caregiver support, and equipment needs.
  • Review and clarify physician orders to ensure accuracy and appropriateness for therapy services.
Qualifications For Occupational Therapists (OTs)
  • Graduation from an ACOTE-accredited Occupational Therapy program.
  • Active Massachusetts OT license (required)
  • Successful completion of the NBCOT exam.
  • Minimum of one year of clinical experience (home health preferred).
For Certified Occupational Therapy Assistants (OTAs)
  • Graduation from an ACOTE-accredited OTA program.
