Job Overview
We are seeking a highly skilled and compassionate Home Health Lead Nurse to oversee patient care operations for our home-based services in Greenwich, CT
. The ideal candidate will have strong clinical expertise in community-based nursing, chronic disease management, geriatrics, and post-acute care. The Lead Nurse will coordinate field nursing staff, ensure compliance with home health standards including CMS, HIPAA, and CT DPH regulations
, and drive high-quality outcomes for patients transitioning from hospitals, SNFs, and outpatient settings.
Experience with home health EMR systems such as Well Sky or Homecare Homebase (HCHB) is essential. This role offers an opportunity to lead a dedicated team committed to excellence in in-home patient care.
- Provide clinical leadership and supervision to home health nursing staff delivering skilled care across Greenwich and surrounding areas.
- Oversee in-home assessments, vital signs monitoring, medication reconciliation, and patient safety evaluations.
- Perform and guide staff in wound care, IV therapy, injections, catheter/ostomy care, and chronic disease management.
- Manage complex home-based cases including dementia care, palliative/hospice support, infection control, and post-acute recovery.
- Coordinate care transitions and ensure compliance with CMS Conditions of Participation
, HIPAA, and home health quality standards. - Use EMR/EHR systems such as Well Sky or HCHB for accurate documentation, OASIS assessments, and coding (ICD-10; CPT).
- Lead infection prevention initiatives and maintain adherence to home‑based sterile technique and safety protocols.
- Provide patient and caregiver education on health coaching, medication management, nutrition, pain control, and dementia support.
- Train staff on procedures such as wound management, tube feeding, home ventilator support, and infection control practices.
- Collaborate with interdisciplinary teams for case management and discharge planning to optimize patient outcomes in the home setting.
- Currently licensed to practice as a Registered Nurse in the State of Connecticut.
- Extensive clinical experience in home health, community nursing, long‑term care, or post‑acute care settings.
- Expertise in managing complex home‑based cases involving chronic disease, dementia, palliative care, and patients with disabilities.
- Proficiency with home health EMR/EHR systems (
Well Sky, HCHB
) and knowledge of ICD-10/CPT coding. - Strong experience completing, reviewing, and ensuring accuracy of OASIS SOC/ROC/Recert/DC assessments in compliance with CMS guidelines.
- Background in geriatrics, post‑acute care, outpatient follow‑up, or assisted living/home care environments is highly desirable.
- Demonstrated leadership skills in supervising field nursing teams and coordinating care in dynamic home health environments.
