Responsibilities :
- Conduct in-home evaluations and treatments for outpatient physical therapy patients.
- Create tailored treatment plans to enhance mobility and recovery.
- Maintain clear documentation and patient communication.
Requirements :
- Licensed Physical Therapist or Occupational Therapist (PT or OT) in Arizona (or eligible).
- Minimum 1 year of outpatient or home health related experience preferred.
- Reliable transportation for travel within the Phoenix metro area.
- Self-motivated with excellent interpersonal skills.
