Position: Licensed Vocational Nurse (LVN) - Community Health (CalAIM ECM Program)

Overview

Licensed Vocational Nurse (LVN) - Community Health (CalAIM ECM Program) at Pacific Health Group.

Base pay range

$30.00/yr - $35.00/yr

Pay: $30.00 - $35.00 per hour

Expected

Hours:

Monday through Friday 8:30AM to 5:00PM (40 hours per week)

Work Location:

Hybrid remote in Santa Clara County, California

Responsibilities
  • Provide direct nursing care to clients in community settings
  • Assist in the assessment and evaluation of clients' health needs
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Monitor and document clients' health status and progress
  • Educate clients and their families on healthcare, wellness, and preventive measures
  • Coordinate with multidisciplinary teams to ensure a holistic approach to care
  • Support clients in accessing mental health and social support services
Qualifications
  • MUST reside in Santa Clara
  • Current and valid LVN license in the state of practice
  • Minimum of 1 year experience in a healthcare or community health setting
  • Strong understanding of mental health care principles and practices
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team-oriented environment
  • Compassionate and patient-centered approach to care
  • Willingness to travel within the community as needed
  • Possess a valid California Driver's License (Class C minimum), maintain a personal, operable vehicle for daily business use, and carry current liability insurance that meets California's minimum legal requirements. All selected candidates will be required to pass a Motor Vehicle Report (MVR) background check prior to employment
