Overview

Care Coordination and Support:
High Fidelity Wraparound (CCSW) supports children with a mental health diagnosis and their families through facilitation of Child and Family Team Meetings, coordination with professionals, and helping the child meet their goals. CCSW provides care coordination to children stratified into Tier
1. Designated CCSW Care Coordinators work with an average of 10 Pathways families at a time and are not assigned to more than 12 families concurrently. The CCSW helps families develop a positive view of their future and uses a strength-based empowerment model to improve functioning in the home, school, and community.

Job Purpose:
The CCSW takes primary responsibility for making the care coordination process happen for children with a mental health diagnosis and their families through facilitation of meetings and coordination with professionals.

• Perform outreach and engagement to locate, engage, and educate Pathways youth and their families. Outreach is required 3 times a week for 60 days or until enrollment or declination of Pathways services.
• Utilize a trauma-informed approach to engage children/youth with significant behavioral health needs and their family/caregivers, connecting them to resources within the community for their assigned caseload.
• Provide intensive care coordination using a strengths-based approach to safety planning, development of family teams and support systems, and wraparound planning to maintain children in their homes, schools, and communities.
• Schedule, plan, and facilitate Child & Family Team Meetings.
• Build and maintain knowledge of available community resources and help link youth and families to needed supports.
• Provide regular communication and close collaboration with multiple community partners.
• Use a system of care approach to coordinate services from community resources, placement providers, collateral agencies, the court, and other partners with families and clients.
• Facilitate the creation of safety and crisis prevention plans.
• Collaborate with local MCR agencies as needed (including Ada S. McKinley’s MCR team).
• Enact Ada S. McKinley Care Coordination Model with each individual and family.
• Facilitate the application process and obtain consents for SFSP/FSP for eligible youth and their families; provide care coordination services to SFSP/FSP eligible youth and families.
• Complete service documentation in alignment with agency and program core performance standards.

• Help families find services and supports in the person-served community or natural environment.
• Maintain good writing skills to complete required documentation.
• Demonstrate strong organizational skills and be a self-starter and multitasker.
• Provide exceptional customer service.
• Manage a large caseload (one to twelve families, depending on population).
• Prepare detailed documentation of activities, including opening/closing electronic records, completing required assessments, creating/updating Wraparound Plans, and ensuring access to Outlook calendar and correspondence.

• Education:
  Bachelor’s degree in social work, counseling, rehabilitation counseling, vocational counseling, psychology, pastoral counseling, family therapy, education or related field; or in any other field with two years of supervised clinical experience in a mental health setting required.

• Job Knowledge, Skills & Experience:
  Experience working with children/adolescents and families is required. Experience with care coordination is a plus. Excellent communication, organization, presentation, and PC/software skills (Microsoft Office: Outlook, Word, Excel, PowerPoint) and related software. Bilingual is preferred.

• Driving:
  Valid Illinois Driver’s License in good standing and a vehicle required; proof of valid auto insurance.
